Impact Aid Survey Forms

These forms went home on Monday, November 14th.

They were due back by Friday, November 18th. 

The Federal Law is specific on the information required
on Impact Aid Survey Forms.

All forms must have the following...

 
                           >  Name of the Student

                           >  Date of Birth

                           >  School Name and Grade

                           >  Parent Signature

We are required to get a signed form 
back from every student. 

If you have not already turned your child's form in, please do so on Monday.
